Open Rhinoplasty (External Approach)

In open rhinoplasty, the surgeon makes incisions inside both nostrils that connect across the columella — the narrow strip of tissue separating the two nostrils. This allows the nasal skin to be lifted entirely, exposing the underlying cartilage and bone in direct view.

Closed Rhinoplasty (Endonasal Approach)

In closed rhinoplasty, every incision stays inside the nostrils. Nothing is visible from the outside. The surgeon works through a more limited field — but as a 2025 academic analysis published in The Egyptian Journal of Otolaryngology notes, the closed technique preserves the columella and maintains the three-dimensional integrity of the lower third of the nose, modifying structures without disrupting their natural support.

The technique demands experience. The narrower working space is not a shortcut — it’s a discipline. In skilled hands, it produces results that are clinically equivalent to the open approach, with a meaningfully faster recovery and zero external scarring.

What Does the Research Actually Show?

Here’s where the conversation gets interesting — and where a lot of online claims fall apart.

A systematic review and meta-analysis published in Plastic and Reconstructive Surgery – Global Open analyzed 20 studies covering 1,067 patients. The conclusion: no statistically significant differences between open and closed rhinoplasty in ROE scores, NOSE scores, swelling, bruising, complication rates, or operative time.

What this means in plain terms: Both techniques work. The real variable isn’t the incision location — it’s the surgeon performing it. Before asking “which technique is better?”, a smarter question is: “How many cases has this surgeon completed with this specific approach, and what do the outcomes look like?”

The Case for the Closed Technique

A study published in Cureus directly compared scar-related quality of life between open and closed septorhinoplasty patients and confirmed that the endonasal approach eliminates scar-associated concerns entirely — which, for many patients, is the single most anxiety-inducing part of the decision.

A single-surgeon audit of 238 consecutive closed rhinoplasty cases (Archives of Plastic Surgery, 2022) reported that an independent blinded panel of board-certified surgeons rated outcomes as “overwhelmingly positive” — with a complication rate of just 0.8%.

Does Rhinoplasty Actually Change How You Feel?

In a 380-patient cohort study, the subjective nasal appearance score rose from 3.2 to 7.9 out of 10 by the six-month mark — a 147% improvement in self-perception. Quality of life and body image scores followed the same trajectory.

Using the validated FACE-Q instrument, a separate PubMed study recorded a +26.5-point gain in satisfaction with facial appearance and a +15.7-point improvement in psychological well-being.
